SHORTEN your in office visit by completing our Patient Form MEDICAL RELEASE (as needed) ahead of time! You know what its like. You arrive at the doctors office with the expectation of getting in and out in a hurry. Then you are asked to fill out all the forms, which can take up to 10 mins.
 

As the Patient, please understand this is your responsibility to call your insurance prior to a blood draw. This BLOOD DRAW INFO form should be consulted with your insurance for blood draws that are not covered. Please sign and bring with you at time of blood draw.

If you are a Parent or Guardian of a child who recently turned 18 or is in college, there is one essential task that is often overlooked: Ensuring the proper KEY LEGAL DOCUMENTS are in place for you child now that they are officially an adult.
